The purpose of this three-part forms package is to provide the Owner/Agent with a process for handling requests for reasonable accommodations relating to Live-In Aides (sometimes referred to as caregivers). State and federal fair housing laws require housing providers to make reasonable accommodations for individuals with disabilities. A reasonable accommodation is a change in rules, policies, practices, or services so that a person with a disability will have an equal opportunity to use and enjoy a dwelling unit or common space.  Form Packet 65.0 can be used to document the request and, if necessary, verify the need for a live-in aide as a reasonable accommodation. If the request for a live-in aide is granted, use Form 65.1 - Live-In Aide Policy Addendum as an addenda to your rental/lease agreement. The packet also includes a sample addenda of the rules and regulations regarding the live-in aide.

A Landlord can use this form to respond to a defective notice of intent to vacate. Depending on the type of defect, the form can be used to notify the Resident that either that their defective notice is accepted (and the terms of that acceptance) or that their notice is not accepted and a proper notice must be provided.

DO NOT USE THIS FORM WITHOUT FIRST REVIEWING THE INSTRUCTIONS CAREFULLY.

If the Resident provided a proper 30-day notice, do not use this form. Instead, use Acknowledgement of Resident’s Thirty-Day Notice to Vacate – Form 21.1.

California law, Government Code 8589.45 requires “every lease or rental agreement for residential property entered into on or after July 1, 2018” to include a specific flood hazard notice. Provision of the notice absolves the landlord from having to provide additional information regarding flood hazards.

This form allows the Landlord to obtain the advantages of providing the flood hazard notice prior to lease renewal. CAA’s Rental/Lease Agreements have included the required provision since January 2018. If your Rental/Lease Agreement already contains the required flood hazard notice, there is no need to use this form.
